Scan Health Care

Whether you are comparing Medicare health plans or have a relative that is in search of better Medicare benefits, SCAN may be your best resource for tools and information to help you make informed health plan decisions. It takes time to make the right choice. Visit the Medicare website so you can compare benefits. You'll find that SCAN offers everything Medicare does.

If you're ready to take advantage of the many benefits SCAN offers, you can call them toll-free at 1-877-452-5898. (TTY users: 1-800-735-2929). 8:00 a.m. - 8:00 p.m., 7 days a week. (PST). *Se habla EspaƱol.

A SCAN representative can answer any questions you may have or can schedule an appointment to come to your home to assist with the enrollment process. You can also submit a request to have a representative contact you or to request additional information.

SCAN Health Plan is a Medicare Advantage Organization with a Medicare Contract. Individuals must have both Part A and Part B to enroll. Benefits, pharmacy network, premium and/or co-payments may change on January 1, from year to year. You must continue to pay your Medicare Part B premium. Members may enroll in the plan only during specific times of the year. Limitations, co payments, and restrictions may apply. Contact SCAN Health Plan for more info.

Medicare Beneficiaries may also enroll in SCAN Health Plan through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov/.
