A SCAN representative can answer any questions you may have or can schedule an appointment to come to your home to assist with the enrollment process. You can also submit a request to have a representative contact you or to request additional information.

SCAN Health Plan is a Medicare Advantage Organization with a Medicare Contract. Individuals must have both Part A and Part B to enroll. Benefits, pharmacy network, premium and/or co-payments may change on January 1, from year to year. You must continue to pay your Medicare Part B premium. Medicare Beneficiaries may also enroll in SCAN Health Plan through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov/.
